I accept BCBS-PPO, Aetna, Cigna and United Health/Optum. I am out of network with Medicare, Medicaid, and BCBS-HMO. I can provide a superbill for those using their OON benefits for claim reimbursement. I also offer sliding scale rates from $100-$160 for those OON or without insurance.
